Analyse server logs to discover informations about bad server setup.
Never copy or generate server.cfg, only add cvars which are necessary. You will know what they do in case of problems. Less cvars is always better than too much.
Always keep updated your plugins so you decrease potentials bugs and problems.
If your server is down, first thing to do is to test it without any plugins, easiest thing is to rename addons folder. If server is back online, then you have to reactivate plugin one by one to find problem origin.
If you must use metamod for one of your plugins, then use metamod version of your others plugins if possible.
Avoid redondancy, especially for configuration files.
To avoid multiplication of useless files, delete binaries you don't use
Do traceroute in case of ping, choke or loss problems to detect eventual network problems.
Reboot is always better after a configuration change.
Every informations you need are on internet, best move is to use Search function on forums and sites and keep in mind Google is your best friend.
Make regular backup of server setup.
